  1. Patient Demographics Entry. This would include Patient's Name, Present Address, City, State, Zip Code, Social Security Number, Employer Details, Insurance Details like Primary, Secondary and Tertiary, and guarantor information. So we enter the details, as provided by your office, on all patients entered into the billing system.
  2. Charge Entry
    This would include entering details like Date of Service, Referring Physician, Ordering Physician, Place of Service, Type of Service, CPT Codes, ICD Codes, Modifiers, Authorization or Referral Details and Co-pay Details.  So we enter all Super bills/encounter forms into the medical billing system.
  3. Payment Entry or Charge Posting
    The following details from the EOB are keyed in-
    Patient Account Number / Patient Control Number, Patient Name, Date of Service, Procedure Code, Billed Amount , Allowed Amount, Adjusted Amount, Paid Amount, Deductibles and Denial Details. So we post all payments that are received by your office. Checks are sent directly to your office and your office staff is required to send the copy of the EOB or check to our office so our medical billing specialists can post the payments to the medical billing system.
  4. Accounts Receivable and Claims Follow-up
    •    Denials/rejections analysis, re-billing
    •    Systemic A/R projects, re-billing

  5. Transmit electronic medical billing claims and/or mail paper claims
    We transmit medical billing claims electronically where possible. If the medical insurance company doesn’t accept electronic claims, we print the claim and mail it to the insurance company.

Privacy is the most talked about ruling. These rulings affect all health care records. The Privacy Rule requires covered entities that transmit any health information in electronic form to maintain the confidentiality of all individually identifiable health information.
